You are a licensed audiologist, hearing aid dispenser, ENT, or medical provider authorized to purchase and dispense prescription hearing aids in your jurisdiction.
You are not a consumer or patient attempting to purchase prescription hearing aids for personal use.
You understand that the sale of prescription hearing aids to unlicensed individuals is prohibited by law and may subject violators to legal penalties.
